From Crisis Response to Upstream Prevention:
How Community Health Workers Build Healthier Communities
Lead Presenter: Rev Dr. Ali ABY Muhammed
This workshop will present a Massachusetts-based, community-engaged prevention model led by Community Health Workers (CHWs) working across community, shelter, behavioral health, and hospital-connected settings in Boston and Cambridge over the past several years.
The session focuses on what happened when frontline prevention work was designed not only to respond to crisis, but also to interrupt the conditions that place people at risk in the first place. The workshop will describe how CHWs used trauma-informed engagement, harm reduction, care coordination, and partnership building to support people affected by housing instability, substance use, mental health stress, social isolation, and barriers to care.
